NAD⁺ vs NAC: two acronyms, two completely different molecules

NAD⁺ and NAC get compared because the letters look alike. One is a coenzyme in energy metabolism; the other is a glutathione precursor and an approved hospital antidote. Here is what each actually does.

These two get compared constantly, and the comparison mostly exists because the acronyms look alike. NAD⁺ and NAC are not competing versions of the same thing. They are different molecules, in different pathways, with very different regulatory standing — and only one of them is a drug with an approved medical use.

What each one actually is

NAD⁺ is nicotinamide adenine dinucleotide: a coenzyme present in every cell, which carries electrons through the reactions that extract energy from food and fuels the sirtuin and PARP enzymes involved in DNA repair. It is not an antioxidant, and it is not an amino acid. If you want the full picture, that is our explainer.

NAC is N-acetylcysteine: a modified form of the amino acid cysteine. Its job is to be a building block. Your body uses cysteine to make glutathione, the main antioxidant inside your cells, and cysteine is usually the ingredient in shortest supply — so supplying more of it can raise glutathione.

So the honest one-line answer: NAD⁺ is about energy metabolism and DNA repair; NAC is about glutathione and oxidative stress. Neither substitutes for the other, and taking one does not give you the other.

The difference that actually matters: one is a real drug

This is the part that gets lost, and it is the strongest reason not to treat them as two flavors of the same supplement aisle.

NAC is the established treatment for paracetamol — acetaminophen — overdose, one of the most common causes of acute liver failure worldwide, and it works by restoring the glutathione the overdose depletes [1]. It is stocked in hospitals and given intravenously in an emergency. It is a medicine with an indication, a dose and a mechanism that is not in dispute.

NAD⁺ has no comparable approved use. The injectable and IV forms sold through telehealth are compounded — not FDA-approved, and not reviewed by the FDA for safety, efficacy or quality before they reach you — and the oral forms are supplements. That is not a slight against the molecule; it is simply a different regulatory position, and worth knowing before you treat both as equivalent purchases.

What the trials show for each

For NAD⁺, the picture on this site is consistent: oral precursors reliably raise blood NAD⁺ in randomized trials, while the benefits they are marketed for — energy, focus, slower aging — are not established in controlled human outcome trials, in any format.

For NAC in a longevity context the evidence is also narrower than the marketing. The most-cited study is a randomized, placebo-controlled trial of GlyNAC — glycine plus NAC — in older adults, which randomized 24 older adults — GlyNAC (n = 12) or an isonitrogenous alanine placebo (n = 12), for 16 weeks — alongside 12 young adults who received GlyNAC for two weeks[2]. It reported improvements across glutathione, oxidative stress, mitochondrial measures, inflammation and physical function.

Read the size before the conclusion. Twelve people per arm is a pilot, and it tests a combination, so it cannot tell you what NAC did on its own. It is a genuine and interesting result. It is not “NAC reverses aging”, and any page telling you it is has skipped the methods section.

Can you take both?

Plenty of people do, and mechanistically they do not tread on each other — different pathways, different jobs. But that is a question for the clinician who knows your medications, not for a website, and NAC in particular interacts with things and has real clinical effects.

What we will say is that stacking two supplements because both are associated with aging is not a strategy. If you have a reason to want glutathione support, NAC is the one with the pharmacology. If you want to raise NAD⁺, an oral precursor is the form with the human data, and it costs a fraction of an injection.

NAD⁺ is a coenzyme whose precursors demonstrably raise its level and whose advertised benefits remain unproven. Choose based on what you are actually trying to do — and treat any page that ranks them against each other as one that did not check what they are.
