Before you can argue about whether a NAD⁺ shot makes you feel anything, there is a smaller question that has to be settled first: does injecting it into the fat of your abdomen actually raise the NAD⁺ in your blood?
Everyone assumes the answer is yes. It is the entire reason to pay four times the price of a capsule. And no one has published the measurement.
The chain has two links, and both are open
Every shot benefit claim — energy, focus, recovery, sleep, slower aging — depends on two things being true in sequence. First, the injection has to raise your systemic NAD⁺. Second, raised NAD⁺ has to produce the effect. Sellers argue the second by pointing at biochemistry and skip the first entirely, because it feels too obvious to check.
Link one. On September 13, 2026 we searched PubMed for subcutaneous NAD+ pharmacokinetics plasma humans. It returned two records, neither of which concerns NAD⁺ — one is about hops and detoxification enzymes, the other a 1991 study of breast tumor xenografts in mice. There is no published pharmacokinetic study of subcutaneous NAD⁺ in people. Nobody has drawn blood before and after a shot and reported the curve.
That matters more than it sounds, because the one time anyone measured what happens to NAD⁺ given by needle, the result was counterintuitive. A pilot study infused NAD⁺ intravenously at 3 µmol per minute for six hours and sampled plasma and urine throughout. No change in plasma NAD⁺ or any measured metabolite appeared until after two hours; the authors describe the infused NAD⁺ as being “rapidly and completely removed from the plasma for at least the first 2 h” [1]. That was a vein, with the dose going in continuously. A shot into subcutaneous fat is a slower, less direct route, and it has never been measured at all.
Link two. Three further searches, same day, same database. Intramuscular NAD+ injection outcome trial adults: zero results. NAD+ injection mental clarity cognition randomized: zero results. NAD+ shot weight loss randomized trial: zero results.
What clearing link one looks like when someone bothers
The oral precursors went through this properly, which is why the contrast is useful. A randomized, multicenter, double-blind, placebo-controlled trial gave 80 healthy middle-aged adults (n = 80) placebo or 300, 600 or 900 mg of NMN once daily for 60 days and drew blood at day 30 and day 60. NAD⁺ rose significantly in every NMN group at both time points, against placebo and against baseline — all p ≤ 0.001 [2].
That is what step one looks like when a product is asked to prove it: a dose, a control arm, a blood draw, a number. A capsule that costs about forty dollars a month has met that bar. A shot that costs $93 to $249 a month has not been asked to.
The benefits, one at a time
Energy. Not measured for a shot. The mechanism is real — NAD⁺ carries electrons through the reactions that release energy from food — but a molecule’s job in a pathway is not evidence that adding more makes a person feel energetic.
Mental clarity and focus. Zero results for injected NAD⁺ and cognition. Where cognition has been measured — in a placebo-controlled randomized trial of 20 older adults with mild cognitive impairment, given nicotinamide riboside at 1 g a day for 10 weeks — blood NAD⁺ rose 2.6-fold (p < 0.001, 95% CI [17.77, 43.49]) and, in the authors’ own summary, NR “did not alter cognition” [4]. The biomarker moved. The thing you would notice did not.
Weight loss. Zero results. This is worth flagging separately because it is the claim most often attached to compounded shots sold next to GLP-1s — the evidence there is its own story and it is not a good one.
Recovery and performance. The nearest real result is an oral one: in a six-week randomized, double-blind trial of 48 amateur runners, NMN alongside training raised oxygen uptake and power at both ventilatory thresholds more than placebo at the medium and high doses — but VO₂max, O₂-pulse and peak power did not differ [5]. A capsule, in people already training five to six times a week, on some endpoints and not the headline one.
Anti-aging. A 2026 PRISMA-guided systematic review of 113 studies states plainly that no eligible outcomes trials evaluated intravenous or intramuscular NAD⁺ itself for anti-aging or wellness indications [3]. Formal reviewers went looking. There was nothing to include.
The flush is not the benefit
Most people feel something after a NAD⁺ shot: warmth, flushing, a tight chest, sometimes mild nausea. It is common enough that providers manage it by starting low, and it arrives fast enough to feel like proof.
It is not proof of anything. No published work establishes any relationship between the sensation and an outcome, for the simple reason that no published work has established an outcome. A drug you can feel is a drug that is doing something; whether that something is the thing you paid for is a separate question, and an unanswered one. What we would ask before buying
Two questions settle most of it, and neither is about benefits. How many milligrams is each shot, and how many shots does the monthly price cover? Most sellers answer neither, which makes their prices formally incomparable — our price table marks who discloses and who does not.
A shot delivers NAD⁺ without passing it through your digestion. That much is true and nobody disputes it. What has not been shown is that the delivery reaches your bloodstream in a measured way, or that reaching it produces the list on the landing page. Two links, both open, and you are being charged for a chain.
