NMN before and after: what the trials measured, and why none of it photographs

NMN has a real randomized trial base — blood NAD⁺, walking tests, sleep scores, about two kilograms of body weight. Almost none of it would show in a photograph, and two PubMed searches for a trial that measured appearance returned nothing.

The reason you cannot find a convincing NMN before-and-after is not that the research is missing. NMN has more randomized human trials behind it than most supplements ever get. The reason is that almost nothing those trials measured shows up in a photograph.

That is a more useful fact than a gallery, so this page gives you the endpoints instead — what was measured, how much it moved, and which of it a camera could ever have caught. What NMN trials actually measured

Blood NAD⁺. The headline endpoint in nearly every study. In a randomized, double-blind, placebo-controlled trial of 80 healthy middle-aged adults (n = 80) on 300, 600 or 900 mg daily for 60 days, blood NAD rose significantly in every dose group at day 30 and day 60 against both placebo and baseline (all p ≤ 0.001) [1]. Invisible, by definition.

Walking. The same trial found the increase in six-minute walking distance significantly greater than placebo in all three dose groups (all p < 0.01) [1]. A 12-week trial of 250 mg daily in 60 older adults is more sobering: its pre-specified primary outcome, a stepping test, showed no significant difference from placebo at 4 or 12 weeks, with a shorter 4-meter walking time and improved sleep quality scores arriving as secondary outcomes [2].

Grip and gait. A trial of 250 mg daily in healthy older men reported what its own authors called nominally significant improvements in gait speed and left grip, requiring validation in larger studies, and no significant effect on body composition[3]. When those outcomes were pooled across trials in adults over 60, NMN showed no significant effect on skeletal muscle index (mean difference −0.42, 95% CI −0.99 to 0.14, p = 0.14) or gait speed (mean difference −0.01, 95% CI −0.08 to 0.06, p = 0.79) [4].

Aerobic capacity. In 48 amateur runners on 300, 600 or 1,200 mg daily for six weeks, oxygen uptake and power at both ventilatory thresholds rose more than placebo in the medium and high dose groups — while VO₂max, oxygen pulse and peak power did not differ in any group [5].

The one endpoint a camera might have caught

Body weight, once, in one trial, by a small amount. Thirty overweight or obese adults aged 45 and over took a microcrystalline β-NMN preparation at 1,000 mg twice daily for 28 days. Body weight fell significantly more than placebo — a difference of −1.9 kg, reported as −3.3 to −0.5, P = .008 — along with diastolic blood pressure, total cholesterol and LDL cholesterol. Muscle strength, aerobic capacity, insulin sensitivity and intra-abdominal fat did not differ between groups [6].

Just under two kilograms, in overweight adults, over four weeks. That is a real, statistically significant result and it is nothing like the transformations in a testimonial gallery. It would be invisible in a photograph taken from the same angle in the same light — and testimonial photos are never taken in the same light.

Nobody has photographed NMN

We looked for the study that would justify a before-and-after page, and the searches are worth stating precisely rather than summarizing. On September 13, 2026, a PubMed search for nicotinamide mononucleotide skin appearance randomized controlled trial returned zero results, and a search for nicotinamide mononucleotide photographs before after trial returned zero results. As a control, a search for nicotinamide mononucleotide randomized controlled trial returned 32 — so the instrument was working when the other two came back empty.

No published randomized trial has assessed what NMN does to how you look. Every image presented as an NMN before-and-after therefore rests on the same foundation as any other testimonial: one person, two photographs, chosen by whoever is selling.

What a testimonial cannot control for, specifically

Everything that changes in a person’s life over the weeks between the two pictures — and in this category, one thing in particular. People who start an NMN course have usually just decided to take their health seriously, which means the supplement arrives in the same month as better sleep, a gym membership and a shorter drinking week. The capsule gets the credit for a bundle.

Controlled trials exist to separate the capsule from the bundle. When somebody did that with NMN, they got the endpoints above: a reliable biomarker, mixed walking results, a primary outcome that missed, and a couple of kilograms in people who were overweight to begin with.

How to run your own before-and-after properly

If you want to try it anyway, that is a reasonable personal decision — and there is a version of it that will actually tell you something.

Pick a dose used in a published trial rather than whatever the label suggests; we list every dose researchers administered. Decide before you start what you are measuring and write the number down — 8 to 12 weeks is the duration the trials used. Measure something objective: a timed walk, a repeatable lift, a lab panel you already get. And know what you are paying for the experiment, which the cost calculator will tell you before you commit.

Are NMN before-and-after photos real?
Some of the people in them are certainly real. What the photos cannot establish is that NMN caused the change, because nothing controlled for everything else that happened in those weeks, and because the pairs that showed no change were never published. No randomized trial of NMN has assessed appearance at all: PubMed searches on September 13, 2026 for an NMN trial measuring skin appearance, and for one using before-and-after photographs, each returned zero results.
What results did NMN actually produce in trials?
Blood NAD⁺ rose significantly at every dose tested in an 80-person trial, at p ≤ 0.001. Six-minute walking distance improved more than placebo in that same trial. In a 12-week trial at 250 mg the pre-specified primary outcome, a stepping test, showed no significant difference from placebo. Pooled across trials in adults over 60, NMN showed no significant effect on muscle mass, grip strength or gait speed.
Does NMN cause weight loss you could see?
One trial found a real but small change. In 30 overweight or obese adults aged 45 and over, 1,000 mg twice daily for 28 days reduced body weight by 1.9 kg more than placebo (reported as −3.3 to −0.5, P = .008), along with diastolic blood pressure and cholesterol. Just under two kilograms over four weeks would not be visible in a photograph.
How long before you would notice anything on NMN?
The trials ran 4 to 24 weeks, and blood NAD⁺ typically rises within the first few weeks. Whether you would notice anything is a different question — the endpoints that moved were mostly laboratory measurements and timed physical tests, not sensations. Three separate meta-analyses found no significant effect on the metabolic and muscular outcomes people take it for.
How should I test NMN on myself?
Use a dose that matches one used in a published trial, give it the 8 to 12 weeks the trials ran, and decide in advance what you will measure — a timed walk, a repeatable lift, or a lab panel you already have a baseline for. Deciding afterwards whether you feel better is answered by your sleep and your expectations long before it is answered by a capsule.
