There is no NAD⁺ injection dosage chart worth having, and this page is not one. What exists is two sets of numbers that are usually kept apart: the milligrams sellers print on a checkout page, and the milligrams published trials have actually administered. Put them next to each other and the gap is the story.
Everything below is reported, attributed and not a recommendation. How much NAD⁺ is appropriate for you is a question for a clinician who knows your history, and it is the one question a website cannot answer.
What the sellers publish
Start with the uncomfortable part: most do not publish anything. Across the injection providers on our injection board, the common pattern is a monthly price with no milligram figure attached at all, which makes the product impossible to compare with any other product, including a cheaper one from the same clinic.
Among those that do state a quantity, the figures we have recorded are:
- System Labs — a stated 1000 mg monthly supply at $149, which is about 15 cents a milligram.
- iThriveMD — a stated 1000 mg a month at $249, around 25 cents a milligram.
- Eden — a 500 mg vial at $186, with a 1000 mg option also listed.
- The IV Doc — a 250 mg starting infusion at $799 per session, with 250 mg, 500 mg and 750 mg add-ons.
Those units are not interchangeable. A monthly supply, a single vial and one infusion session are three different things, and reading “1000 mg” against “250 mg” without noticing which is which is the easiest mistake in this category. What the trial literature used
For NAD⁺ given parenterally — as an injection or a drip — the published human record is thin enough to describe in a paragraph.
The one randomized, placebo-controlled trial of intravenous NAD⁺ we located enrolled 180 adults with heart failure from ischemic cardiomyopathy and gave 10 mg a day intravenously for seven days alongside guideline-directed medical therapy. At one month the NAD⁺ group had a greater improvement in left ventricular ejection fraction than placebo (45.44 ± 8.55% vs 42.44 ± 9.09%, p = 0.024); the six-month composite event rate was lower but not statistically significant (14.6% vs 24.7%, p = 0.089) [1]. That is a cardiology trial in sick patients, not a wellness protocol — and its daily dose is a small fraction of a single wellness infusion.
The other relevant human study is a pharmacokinetic pilot that infused NAD⁺ intravenously at 3 µmol per minute over six hours and tracked the plasma and urine metabolome. Plasma NAD⁺ and its metabolites did not change until after two hours, and the authors concluded the infused NAD⁺ was being rapidly and completely removed from plasma for at least those first two hours [2]. It measured where the molecule went. It did not measure whether anyone felt better.
A systematic review published in early 2026 makes the absence explicit rather than leaving it to be inferred. Searching human and rodent intervention studies of NAD-related compounds from January 2010 to October 2025, it identified 113 eligible studies — 33 of them human intervention studies, 28 randomized — and reported that no eligible outcomes trial evaluated intravenous or intramuscular NAD⁺ itself for anti-aging or wellness indications [3].
Why the chart cannot be built
A dosing chart for an approved drug is an output of dose-ranging and outcome trials: this much produces this effect at this risk, adjusted for weight or kidney function or age. Injectable NAD⁺ has none of that scaffolding. It is compounded rather than FDA-approved, which means the preparation is not reviewed by the FDA for safety, efficacy or quality before it is dispensed, and the wellness indications it is sold for have no controlled outcome trials behind them.
So any chart you find giving milligrams per kilogram, or a weekly titration schedule, was written by somebody who had nothing to derive it from. That is not a small editorial quibble on a drug people inject at home.
The oral side is different, and it is worth knowing that the better- evidenced doses are all on that side: an NMN trial of 80 adults at 300, 600 and 900 mg daily, and a high-dose nicotinamide riboside safety trial at 1,500 mg twice daily for four weeks. The questions that replace the chart
Ask the provider how many milligrams are in the vial, and over what period. Ask whether the figure is per injection, per vial or per month. Ask what the concentration is, because a dose is meaningless without one. Then take those answers to a clinician.
